Application to use the 4-H Name and Emblem

To request permission to use the 4-H Name and Emblem from 4-H National Headquarters, download the Application form (fill-in PDF file). Be sure to read the legal information about use of the name and emblem. NOTE: Because USDA mail is irradiated for security, it is recommended that you use Fax or delivery services as appropriate to send application forms and samples.
